Many people ask why our services cost what they do, how we operate as a not-for-profit, and how we balance financial sustainability with accessibility. This page explains exactly that.

Why we exist

Gender-affirming healthcare should be free, high quality, and available through the NHS. Unfortunately, it’s not.

With waiting times stretching into years and limited access to youth-appropriate support, too many people are left without timely, safe, and affirming care. Anne was created to help fill that gap. Not to replace the NHS, but to offer a community-focused solution that helps people access essential care without waiting years.

We are a not-for-profit organisation. This means we exist to provide gender affirming care, not to generate profit.

Our legal structure

Anne is registered in the United Kingdom as a private company limited by guarantee without share capital. This is a standard legal structure for not-for-profit organisations. This structure means:

  • We have no shareholders
  • No one can take profit from Anne
  • Any surplus money must be reinvested into our mission

Our work is legally bound by our articles of association, which commit us to operating as a not-for-profit organisation.

How our pricing works

Providing safe, high-quality gender-affirming care requires clinical expertise, strong governance, and specialist support. Your membership fees cover the actual cost of delivering care, including:

  • Clinician time with doctors, nurses, and care coordinators
  • Medical oversight and governance
  • Care planning and coordination with partner providers
  • Operational costs such as technology, safeguarding, administration and compliance

We pay our staff fairly, maintain safe clinical standards, and ensure every service meets the level of care our patients deserve.

How we use surplus

Because we are not for profit, any surplus is used to improve access to care. This includes:

  • Subsidising or fully funding care for people who cannot afford our standard fees
  • Developing new care pathways and services
  • Improving systems to deliver care faster, more safely, and to more people

We pay our staff fairly, maintain safe clinical standards, and ensure every service meets the level of care our patients deserve.

A note on F1NN5TER’s involvement

Internet creator F1NN5TER played a founding role by making an initial personal donation that helped Anne get started. They are not involved in the day-to-day running or management of the organisation, and they do not receive any compensation or financial benefit from Anne.

Any ongoing fundraising efforts or donations from F1NN5TER go directly into our funded care programme.

Our ongoing funded care programme

We run a funded care programme for trans+ people who cannot afford our standard pricing. These places are supported by surplus income, individual donations, and community fundraising efforts.

The next application round opens in January, with full details available on our website and social channels. We aim to make the process as fair, accessible, and transparent as possible.

Our commitment to financial transparency

We believe that trust requires openness.

Our first statutory accounts will be filed by May 2026, as required for our first financial year end in August 2025. We are also exploring ways to share financial information sooner, including how funding is used and how subsidised care is allocated.

This is a reasonable expectation from our community, and something we are committed to doing well.

We exist to provide gender affirming care that is accessible, compassionate, and community focused. We do this as a not-for-profit organisation because our mission matters.

  • Your donation will go directly toward funding gender-affirming healthcare for transgender people who can’t afford private care. We’re committed to transparency, and you’ll receive updates about the impact your support is making, including the number of people your generosity has helped.

  • Applications will open in January 2026. At that point, we’ll publish the application form on our website and share it through our social media channels.

    Details will be announced closer to the time.

  • We understand that almost everyone who applies will be doing so in good faith, but it’s essential to prioritise those most in need. To ensure fairness, applications will go through a screening process that includes providing proof of income and benefits.

    A panel of trans and trans-adjacent team members from Anne will carefully review applications. After this, applicants will have an appointment with a member of our therapeutic team to confirm their capacity to consent.

    For privacy and confidentiality reasons, we won’t share the personal details of successful applicants. However, recipients will have the option to provide a short statement confirming their success, helping us demonstrate that all funds are used as intended.

  • We’re prioritising individuals of any age in the UK and Ireland who haven’t been able to access private, gender-affirming healthcare due to financial barriers.

    Applications are open to:

    People who need safe, medically overseen trans supportive healthcare who have been unable to access this due to lengthy waiting times for NHS care, and cannot afford private care. This includes people who are currently self-medicating and parents/guardians applying on behalf of their children.

    Please note: While families can apply for young people under 18, we cannot accept direct applications from minors, as they are unable to enter into a binding legal contract.

    Our goal is to help those most in need, ensuring care is available to those who’ve struggled to access it.
